1. Use Permit and Hold Harmless Agreement (click here for downloadable PDF form).  Please complete and submit this form to the Camp Office prior to using the camp.  Generally, a completed form must be submitted each time the camp is used.   Some organizations have an annual form on file.
2. Camp Check Out form (click here for downloadable PDF form).  Please complete this form when exiting the camp and turn it in with the camp keys.
